Comprehensive Evaluation – $1,500
- Extensive cognitive assessments and results.
- Comprehensive medical and physical intake.
- Complete medical record review.
- Care team meeting, with Dr. Ken Kosik and the complete Cottage Center for Brain Fitness team, with you and your family members (who can join the meeting remotely if needed).
- Personalized Program – for you to use at home or when you join Cottage Center for Brain Fitness. This includes a personalized cognitive exercise program (with computer log-in), physical exercise program, nutritional guidance, stress reduction and more.
Pre-chemotherapy cognitive evaluation – $400
- Serial cognitive testing that begins before starting chemotherapy and will continue at regular intervals during and after treatment.
- Meeting with staff to review test results.
Brain Wellness Package – $600 (Eight 1-hour sessions)
- Weekly sessions of cognitive challenges, exercise and more with our medical staff (therapists, nutritionists, and others).
- Social, spiritual and stress counseling and support.
- Caregiver resources and guidance to help your loved ones.
- Neuropsychological and medical guidance and recommendations.
- Comprehensive cognitive re-assessment (optional).

Clinical Trials for early symptoms of
Alzheimer’s disease

We provide Navigator services to link clients with clinical trials. Discussion of available clinical trials, client suitability, and risks are all discussed. If appropriate, a referral is initiated preferably to a local trials center.
Adult Education Classes
Genetic Testing – $1,800
- Standard genetic testing reports on one’s risk for a set of diseases for which genetic markers are available including Alzheimer’s disease.
- For those with symptoms, testing can be used diagnostically and for those at risk, testing can be used predictively. The package includes pre- and post-test counseling and a complete report.
- More extensive genetic testing for specific rare neurological disorders is available on special order.
