1.  High school diploma
  2. Official high school transcript or GED documentation showing a minimum grade point average of 2.5 (on a scale of 4.0). Students may have High School transcripts waived if college transcripts are provided. College transcripts must show completion of a minimum of 15 semester units of regular college lecture courses with a minimum grade point average of 2.0.
  3. Two letters of recommendation. Letters from instructors and employers are preferred.
  4. Completed application form
  5. Personal interview. Applicants will be notified of interview appointments only after receipt of the application, transcripts, and letters of recommendation.
  6. Physical requirements for the program include the ability to lift 30 pounds, and no restrictions for repetitive bending, lifting, turning, stooping, or walking.

Tuition
A tuition fee of $350 is required and must be paid in full by the first day of class. Tuition is not refundable. Students who do not complete the class forfeit all tuition.

Certification
A certificate is provided upon successful completion of the training program. The certificate fulfills requirements for venipuncture and skin puncture set by the California State Department of Health Services in Section 1246(b) of the Business and Professions Code. The certificate meets the prerequisites for employment as a phlebotomist in laboratories licensed by the California State Department of Health Services.
