CHS provides these insurance benefits at no cost to full-time eligible employees:

Higher levels of coverage are available at low cost. Coverage is available for dependents (spouse and/or children) for a nominal extra cost. For additional information on Cottage's benefits, contact Human Resources.
The following summary is intended only as a brief outline of the employee benefit programs and services available at CHS. More detailed information is available from the Human Resources Department. Benefits are not provided to leased, temporary or agency employees.
MEDICAL INSURANCE
Three PPO plans are available. A PPO plan ensures you have the right to seek treatment from the provider of your choice. The chart below summarizes the main provisions of the medical plans. CHS administers all plans. Current costs for the three medical insurance options are listed here.
Service:
- Annual Deductible (per person): $200 to $600, depending on which plan you select.
- Routine Office Visits PPO (deductible waived): $15 co-pay
- Eye Exams
- Immunizations fully covered
- Vision: 70 to 80% covered
- Emergency Room (all facilities): $50 co-pay
- Inpatient Facilities: 60 to 100% coverage
- Physician Fees (other than routine): 80 to 85% coverage
- Lab/X-Ray: 60 to 100% coverage
- Chiropractic/Acupuncture (20 visits/year max : $25 co-pay
DENTAL
Two plans are available. Preventative care is provided at 90-100%, basic care at 70-80% and major treatment at 50-60%. Level of coverage is based upon choice of provider. While you can use any provider, contract rates with our PPO providers offer savings to both the individual and the plan. CHS administers both plans. WELLNESS BENEFITS:
CHS is committed to both the health of employees and the health of our community. The following Wellness benefits are available up to a maximum of $250 per person up to $500 per family, annually. All reimbursements are subject to the preferred provider contracted rate.
- Fitness Center
- Nutritional Counseling
- Hearing Testing
- Preventive Testing/Diagnostics (mammogram, pap smears, other testing not to treat an illness or injury)
- Massage Therapy (must be performed by a CMT)

PAID TIME OFF (PTO)
Cottage's PTO policy provides eligible non-exempt employees with paid time for planned and unplanned absences including vacations, CHS approved holidays, and for short-term illnesses (up to 16 hours). Full-time and part- time benefit-eligible employees, excluding In-House Travelers, qualify for PTO from the beginning of the pay period following 90-days of employment.
