Post-Hospital Planning

When your physician writes a discharge order on your medical record, you and your nurse will coordinate your discharge time. Staff will provide resources to assist you in a safe transition to your home or another facility and a return to your maximum level of functioning and wellness. If you have concerns, ask your nurse for assistance.
